Onvida Health recently set a new standard in diabetes care by becoming the first acute care hospital in the United States to receive the DNV Advanced Glycemic Management Certification (AGMC). This certification highlights the hospital’s dedication to providing consistent, safe, and compassionate care to patients with diabetes. With this achievement, Onvida Health sets a new standard by helping to prevent complications and keeping patients from returning to the hospital by closely managing their blood sugar levels.
The AGMC certification helps hospitals establish and maintain the highest standards in diabetes care and blood sugar management. To earn it, the hospital demonstrated that it’s a leader in patient safety, patient education, and by training its staff in the best ways to help patients manage their diabetes.
“Being the first hospital in the nation to achieve this certification truly speaks to our commitment to providing the highest level of quality in diabetes care,” “Our team has worked tirelessly to ensure our patients receive consistent, compassionate, and effective care during their hospital stay and beyond.”
Onvida Health was also recently honored with the Arizona Hospital and Healthcare Association (AzHHA) Quality Award for Community Engagement for its impactful program, “Enhancing Diabetes Self-management for Underinsured Low-income Individuals with Diabetes.” Through this program, Onvida Health provided free blood glucose meters to underinsured patients. These meters are small, portable devices used to measure blood sugar at home, which is key to managing diabetes effectively. Through this program, 86% of participants had a HbA1c reduction within 90 days of discharge after receiving a standard blood glucose meter and diabetes self-management education.
“Our mission is to ensure all patients with diabetes deserve access to high-quality diabetes self-management education and the necessary tools to manage their condition effectively at home,” shared Diabetes Care and Education Specialist and program coordinator, Vanesa Orozco.
Additionally, Onvida Health’s diabetes team was honored as the Quality Hall of Fame Grand Champion during this year’s Innovation Week. Guided by what Yuma County residents shared in recent Community Health Needs Assessments, the hospital’s diabetes education team has been hard at work, providing patients with the resources and support they need to manage their health and build healthier futures.
“Our team is truly honored to be recognized for the advancements we’ve made in diabetes care,” said. “Our team is committed to continue to provide the best resources and support to empower our local community to live healthier, more independent lives.”
