Expressing a Compliment or Concern

If you would like to express a compliment, provide thanks, or give kudos in relation to care you received or for a particular care giver, please contact communityrelations@onvidahealth.org.

Additionally, Onvida Health participates in the DAISY Award program which recognizes nursing excellence. You may nominate a nurse for this recognition here.

If you would like to express a concern, we encourage you to let us know how we can serve you better. Most of your concerns can be handled by talking with your physician or caregivers in real time. If you would like additional assistance, we have a Care Advocacy team that can help. You can reach the team by calling 928-336-2002 or by emailing careadvocate@onvidahealth.org. While most of your concerns can be resolved directly with Onvida Health, you also have the right to notify others for assistance.
